Empowering Black Women in Delaware

Black women in Delaware deserve care that understands them. The Connected Communities Recovery Program is here to provide real support for those navigating Substance Use Disorder (SUD) and Opioid Use Disorder (OUD).

We take a holistic, hands-on approach to case management, connecting Black women in New Castle, Kent, and Sussex Counties to the care and resources they need to heal and thrive.

 

How We Support Our Community


Culturally Competent Care

We train our Community Care & Resource Navigators to provide inclusive, bias-free support rooted in Black experiences. We also offer this training to providers and organizations to combat stigma in healthcare.

Personalized Support & Assessments

Every woman’s journey is different. Our navigators take the time to understand each person’s unique needs - including mental health, trauma history, and social factors - to build the right care plan.

Ongoing Recovery & Extended Support

Healing doesn’t happen overnight. Our team checks in quarterly to make sure participants continue to get the help they need.

Building Stronger Communities

We partner with local organizations to create a network of care that is responsive, effective, and truly supportive. Our team hosts quarterly meetings and one-on-one sessions with providers to improve services.

Strengthening Black Maternal Health & Substance Use Movement

Black Mothers In Power is fighting for better support for Black women and girls in Delaware facing Substance Use Disorder (SUD) and Opioid Use Disorder (OUD) at all stages of motherhood.

Through the HEAP initiative, we amplify Black women’s voices, provide education, and push for better access to care. We’ve gathered real stories, built a strategic plan, and created a substance abuse workshop to inform and uplift our community. Recovery is possible, and support should be accessible.
