Substance Use Navigator (Case Manager)
*Looking to hire 4 people: 2 NCC, 1 KC, & 1 SC
Are you passionate about supporting individuals affected by substance and opioid use across our state? We’re seeking a Substance Use Navigator (Case Manager) to join our team. In this essential role, you’ll guide clients through their recovery journey, help them access critical resources, and provide comprehensive support throughout their treatment. You will serve as the key point of contact for individuals facing substance use and opioid use disorders (SUD/OUD), ensuring they receive the care and resources necessary to improve their wellness.
You’ll be part of a supportive, collaborative team where you’ll directly influence positive outcomes for clients by connecting them to treatment programs, social services, and community resources. You’ll work at the intersection of healthcare, social services, and advocacy to help individuals and families affected by substance use challenges.
Key Responsibilities
1. Client Assessment & Needs Evaluation
Conduct thorough assessments of clients to evaluate their physical, emotional, social, and mental health needs, identifying the necessary support and resources.
Engage with clients to understand their unique needs, goals, and barriers to recovery, providing a personalized care plan that aligns with their recovery journey.
Use evidence-based tools and practices to ensure accurate assessments and a comprehensive understanding of client needs.
2. Care Plan Development & Implementation
Develop individualized, recovery-focused care plans in collaboration with clients, ensuring they are centered on the client’s personal goals and recovery milestones.
Monitor progress by regularly reviewing and adjusting care plans based on client feedback, treatment progress, and changing needs.
Collaborate with clients to set achievable, realistic goals while also offering support for short- and long-term recovery success.
3. Service Coordination & Resource Referral
Connect clients to appropriate services, including substance use treatment programs, housing support, financial assistance, healthcare, and counseling services.
Manage and oversee referrals to treatment providers, ensuring timely access to care, and track clients' progress through these programs.
Build and maintain relationships with external service providers to ensure clients receive holistic and coordinated support.
4. Client Advocacy & Empowerment
Advocate on behalf of clients to ensure they have access to needed services, addressing any barriers to care, and empowering them to take an active role in their recovery.
Facilitate communication between clients and service providers, ensuring that clients understand their options and are receiving the best possible care.
Support clients in navigating challenges, providing guidance and reassurance as they work through recovery and reintegration.
5. Documentation & Case Management
Maintain accurate, up-to-date documentation for all client interactions, care plans, and progress updates, ensuring compliance with regulations and confidentiality standards.
Track client progress, document service outcomes, and report on case status as required by regulatory agencies and program funders.
Ensure all records are organized, detailed, and accessible for team collaboration and external reporting requirements.
6. Monitoring & Progress Evaluation
Regularly evaluate client progress toward treatment goals, adjusting care plans when necessary, and ensuring that clients are receiving the appropriate services.
Provide regular updates to program leadership on client progress, service outcomes, and any challenges or concerns.
Offer continuous support and guidance to clients to help them stay on track with their recovery goals.
7. Collaboration & Team Support
Collaborate with team members, healthcare professionals, and community organizations to ensure clients have access to all necessary services and support.
Participate in regular team meetings to share updates on client progress, discuss challenges, and provide input on case management practices.
Support colleagues in providing high-quality case management services and share expertise on best practices for client engagement and recovery.
8. Education & Resource Sharing
Educate clients about treatment options, recovery strategies, relapse prevention techniques, and available resources to help them make informed decisions about their care.
Provide ongoing education and support to clients to help them build coping skills and increase self-sufficiency in their recovery.
Empower clients to access community-based resources and understand the full scope of available services.
Qualifications
Minimum Requirements:
Experience: 1-3 years of experience in case management, substance use treatment, or related social services.
Leadership: Experience working in a case management capacity, with knowledge of substance use disorders and treatment systems.
Communication: Strong communication and interpersonal skills to build relationships with clients, teams, and external service providers.
Care Plan Development: Ability to design, implement, and monitor care plans, with a focus on client-centered outcomes.
Organizational Skills: Strong organizational abilities, with a proven track record of managing client caseloads and documentation.
Documentation & Compliance: Knowledge of regulatory standards and a commitment to maintaining accurate and confidential client records.
Transportation: A valid driver’s license and reliable transportation may be required for some positions.
Desired Skills:
Team Collaboration: Ability to work collaboratively with a team of professionals and community partners to support clients.
Client-Centered Care: Expertise in working with clients from diverse backgrounds, using strengths-based and client-centered approaches.
Advocacy & Empowerment: Advocacy skills to ensure clients receive the resources and care they need for long-term recovery.
Crisis Management: Ability to manage crises and support clients through difficult moments in their recovery journey.
Multitasking & Time Management: Excellent multitasking and time management skills, with the ability to prioritize tasks effectively in a fast-paced environment.
